Treating every patient as our own
treasured loved one…

Why Grace Hospice?

Although all hospices are governed by the same rules and regulations, also called “Conditions of Participation,” not all hospices are the same. Grace Hospice offers the unique advantage of a continued couplesittingonbenchclose relationship with your primary care physician, and also the ability to connect you and your loved one with a physician who offers house calls. Grace Hospice offers a true continuum of care with a focus on quality of life.

Quality of life starts with excellent care provided by our team of physicians, nurses, hospice aides, social workers, chaplains and volunteers. All of our locations are CHAP accredited – one of the most stringent accrediting bodies in the nation. Established in 2008, Grace Hospice has served thousands of patients and families in seven states.

“The care given to our dad by your staff, individual nurses and Chaplain was beyond expectations.
They were always available and every concern and question was answered. We very much appreciate their kindness, thoughtfulness and compassion. We could not have asked for better care for our dad.”
